家庭医生签约服务模式对2型糖尿病患者健康管理的效果分析

摘 要 目的：探讨家庭医生签约服务模式对社区2型糖尿病患者健康管理的效果分析，为提高社区2型糖尿病健康管理水平提供依据。方法：选取截至2014年9月在本社区自愿接受家庭医生签约服务管理的2型糖尿病患者1 282例，其中男性564例，女性718例，平均年龄（67±8）岁。签约后接受家庭医生管理服务2年（2014年10月至2016年9月）后，比较签约前和签约管理服务2年后糖尿病患者的服药依从性和空腹血糖控制情况。结果：签约服务管理后，2型糖尿病患者的服药依从性从签约管理前的72.93%（935/1282）提高到86.35%（1107/1282，P

近10年恚我国糖尿病患病率显著增加，糖尿病流行情况更为严重。2005年到2015年间中国由于糖尿病及相关疾病导致的经济损失达5 577亿美元[1]。而糖尿病复杂的发病过程至今未找到根治的方法，这就意味着患者需要接受终身治疗和健康管理。而家庭医生正逐步发挥着社区健康守门人的作用。本文在家庭医生制服务的基础上，分析家庭医生签约管理2型糖尿病效果，为提高社区2型糖尿病健康管理水平提供依据。

1 对象和方法

1.1 研究对象
