冠状动脉慢血流现象与血浆内皮素的相关性研究

摘要目的:探讨冠状动脉慢血流(SCF)与血浆内皮素水平(ET)的相关分析。方法:2012年8月-2014年2月收治行冠状动脉造影(CAG)提示心外膜主要冠状动脉无明显病变患者84例。采用校正TIMI血流计帧法(cTFC),将冠状动脉平均帧数>27帧者归为SCF,将患者分为SCF组(36例)和对照组(48例)。应用t检验和卡方检验对两组临床资料进行比较,并通过logistic回归分析对ET水平进行分析。结果:两组患者性别、BMI、高血压、糖尿病、高脂血症、吸烟史比例、血红蛋白、血小板、白细胞计数、空腹血糖、甘油三酯及总胆固醇差异无统计学意义,而SCF组ET水平显著高于对照组。Logistic回归分析表明ET是SCF的危险因素。结论:ET水平与SCF有关,可能参与了SCF病理生理过程。

AbstractObjective:To explore the correlation analysis of coronary slow flow phenomenon and plasma endothelin.Methods:The coronary arteriography(CAG) of 84 cases prompted epicardial main coronary arteries with no obvious pathological changes.The 84 cases were selected from August 2012 to February 2014.Using the corrected TIMI frame count(cTFC) of blood flow,the average frame of coronary artery were more than 27 frames as SCF.The cases were divided into SCF group with 36 cases and the control group with 48 cases.The clinical data of the two groups were compared by using t test and chi square test.The ET level was analyzed by logistic regression analysis.Results:The difference of gender,BMI,hypertension,diabetes,hyperlipemia,the roportion of smoking history,hemoglobin,platelet,white blood cell count,fasting blood glucose,triglyceride and total cholesterol of the two groups were no statistical significance.The ET level of SCF group was significantly higher than that of the control group.Logistic regression analysis showed that ET was a risk factor for SCF.Conclusion:The ET level has a relation with SCF.It may be involved in the pathophysiological process of SCF.

冠状动脉慢血流(slow coronary flow,SCF)指经冠状动脉造影(coronary angiography,CAG)未发现心外膜冠状动脉明显病变,却有血流灌注延迟的现象[1]。SCF经CAG检出率约1%,并逐渐被临床重视。近年来研究发现,内皮素(endothelin,ET)水平增高在心血管疾病发生、发展中具有重要意义。本研究采用校正TIMI血流计帧法(Corrected TIMI Frame Count,cTFC)诊断SCF,并分析其与ET水平的相关关系。

资料与方法

2012年8月-2014年2月收治行冠状动脉造影(CAG)提示心外膜主要冠状动脉无明显病变患者84例,并排除冠状动脉痉挛及扩张、冠状动脉成形术后、溶栓治疗后、心脏瓣膜病、心肌病等。采用校正TIMI血流计帧法(cTFC),将冠状动脉平均帧数>27帧者归为SCF,将患者分为SCF组(36例)和对照组(48例)。

SCF诊断标准:校正TIMI血流计帧法(cTFC):以桡动脉为径路,用5F多功能导管,以Judkins法行CAG,多记录在以30帧/秒的速度计数造影剂从各冠状动脉起始端充盈至末端的帧数。其中cTFC第一帧的判断:造影剂完全充盈冠状动脉起始端,见造影剂前向运动;最后一帧的判断:造影剂到达靶动脉末端标记性分支。末梢标记性分支为:左前降支(LAD)-心尖部,若LAD包绕心尖,则最接近心尖的分支为末端标记;左回旋支(LCX)-钝缘支最长动脉的远端分叉处;右冠状动脉(RCA)-左室后支动脉的第一分支。计数时LAD和LCX取右前斜加足位,RCA选左前斜加头位。因LAD较长,以其帧数除以1.7,即为cTFC[2]。

临床指标:所有研究对象入院第2日清晨空腹抽取静脉血,HITACHI 7020全自动生化分析仪测定空腹血糖、甘油三酯、总胆固醇,DANAMHC-2022全自动血球分析仪测定血常规。ET检测采用放射免疫法,所用仪器为γ放射免疫计数仪。

统计学处理:应用SPSS 17.0统计软件处理数据,计量资料以x±s表示,两组间计量资料比较用t检验,计数资料比较用χ2检验。P

结果

入选病例资料比较:两组性别、BMI、高血压、糖尿病、高脂血症、吸烟史比例、血红蛋白、血小板、白细胞计数、空腹血糖、甘油三酯及总胆固醇差异无统计学意义(P>0.05),ET水平差异有显著统计学意义(P

ET相关性分析:非条件logistic回归多因素分析表明,ET(OR=11.081,95% CI 2.186~56.170,P=0.004),ET水平增高是SCF的危险因素,见表2。

讨论

SCF现象机制尚不明确,目前国内外相关研究有限,大量临床实践表明,SCF是冠心病的早期阶段,患者可发生类似心绞痛症状,甚至进展到急性冠状动脉综合征、心肌梗死。

ET是由内皮素前体蛋白生成,分子量2 500,含21个氨基酸残基,在人第六染色体上可检测到其基因[3]。ET广泛分布于各组织器官,在冠状动脉、脑血管及心内膜上含量较高,作为各种炎性递质和激素调节肽参与生理调节[4]。在心血管方面,ET作用于延髓核团,是已知收缩冠状动脉作用最强的递质,是去甲肾上腺素缩血管作用的100倍,并作为单核细胞的趋化因子刺激动脉平滑肌细胞增生紊乱,造成冠状动脉痉挛、动脉粥样硬化及心肌梗死,且ET水平与心肌梗死面积成正相关[5,6]。有研究表明,ET水平异常与高血压、高脂血症有关,并降低血管内壁的顺应性[7]。

本研究初步探讨了可能与SCF相关的因素,发现ET水平升高是SCF的危险因素,提示ET可能参与了SCF病理生理过程。
